Horace Mann is seeking an experienced and compassionate Counselor to support members through ESI’s Employee Assistance Program (EAP). In this role, you will help individuals navigate behavioral health, substance use, relationship and work-life concerns through timely assessment, brief counseling and connections to appropriate care and resources.
You will provide telephone, virtual and, when assigned, in-person support while managing cases from initial intake through follow-up. You will also collaborate with providers, colleagues and client organizations to support member needs and continuity of care.
The ideal candidate brings sound clinical judgment, strong communication skills and a member-focused approach to helping individuals address challenges and take meaningful steps forward.
Key Responsibilities
Clinical Assessment & Counseling
- Conduct assessments and telephonic triage to identify member needs, evaluate concerns and determine appropriate support.
- Provide crisis support, coaching and brief solution-focused counseling through telephone, telehealth and, when assigned, in-person services.
- Make clinical determinations consistent with company risk policies and the best interests of the member.
- Apply knowledge of behavioral health conditions, levels of care and discharge-planning practices to guide care recommendations.
Case Management & Resource Coordination
- Manage cases from initial intake through completion, including timely follow-up and coordination of services.
- Connect members with appropriate network providers and financial, legal, personal assistance and work-life resources.
- Consult with service providers, coordinate referrals and complete session authorizations.
- Collaborate with colleagues and after-hours service providers to support case completion and continuity of care.
Client Consultation & Collaboration
- Consult with Human Resources professionals and supervisory personnel from client organizations regarding employee-related concerns.
- Partner with internal colleagues and external providers to address member needs while maintaining appropriate professional boundaries and confidentiality.
Documentation & Professional Standards
- Maintain timely, accurate and professional documentation within the case management system.
- Handle Protected Health Information (PHI) in accordance with Health Insurance Portability and Accountability Act (HIPAA) requirements and company policies.
